Smart Forms are sent as a Direct Message so you'll need to have both an email address and a mobile number for the intended recipient. To send a Smart Form to a patient:
1. Click on Create -> Form/Assessment
2. Choose which form you'd like to send and click on the Direct Message icon at the end of the row (NB. Do not open the form and then send it to the patient as this will send it in a non editable state) :
3. Select a recipient
4. Enter a message, choose an expiry date and click Send:
5. You'll then see confirmation that your message has been sent:
